Preferred format for additional sheets of paper?

What is the preferred format when attaching additional sheets of paper to certain forms? For example, if there is not enough room on the form for a particular answer, they ask you to attach an additional sheets of paper with the rest of the answer. How do you setup these additional sheets of paper? Do you attach them right at the back of the form when you're done completing that particular form?

You can print by using MS XL or Word format and attach to the respective form. Mark on the top of the sheet : additional sheet for form # IMM ----- .

I would also suggest adding your client ID (if you have one) and/or your names on the page as well. That way if it gets mis-filed or falls on the floor and picked up later, it can be returned to your file.

Wondering where should one attach the additional sheet? at the very end of the form or right after the respective sheet in the form?
