This post is a few days old already, not sure if you got your answer. But if you are in Canada you don't need to fill this form, or at least I did not and was not asked for.

Also regarding your question about Health Care, I think your option for those three months is to get a private insurance.

Do the same as you did for your application but choose the option for the PR Card from the drop down list. As I said, I am still unable to log with this option, I tried with the COPR number and the UCI number, and I guess it's because the process has not started yet.

Not sure what other details I can share, it seems to be as simple as checking the PR process itself. I cannot say if that actually works because I haven't been able to log yet.

i thought it could be something like, you can pay and then apply for reimbursement once you get your health coverage.

Oh... That's right, I've heard that! I had forgotten about it, but not sure if it's applicable to all provinces.

Here http://www.ontarioimmigration.ca/en/after/OI_AFTER_HEALTH.html it says

"To cover the waiting period you should buy private insurance"

what is your PR card number ? how do you know it before even getting the card ?
 
I don't have my PR number, but there are other options to log in into Ecas, like UCI, Copr or application number. Should be the same as when you log to check your PR process and then you can use Receipt number for example.
