Hi all,

I have recently came to canada as PR. I had applied for OHip but while availing the same for my daughter 18 months old, being an US citizen she didn't qualify for that. Now I am in process of applying the PR for her but I am confused with the forms and have few questions as below. I would really appreciate if you guys can respond and help me with this situation -

1) which center I should send the application to? Mississauga or Buffalo
2) how much fees I need to pay?
3) do I need to send the medical and police certificate with the application?
4) do I have to fill Outland application by following forms on guide 3900 and 3999

Thanks Ramakris. My daughter is with me in Canada. Does that still allow me to file Outland application as compared to me inland.
 
what about your spouse Is him/her a PR. If so you have to apply outland.

If not you can apply for both of them together as Inland. This takes little longer process and it cant be appealed if decision against you.
