PR Card not Received Yet- PRTD Timeline In India

Hi Everyone
I got my eCopr in June 2021 but haven't received my PR Card yet. I need to travel to India. I am planning to apply for a PRTD document.
Does PRTD gets rejected too under any circumstances? What reason I should provide inside PRTD application ?
