PR and Open Permit application for Philippines citizen Live-in Caregiver

I have a few questions about the details of the document checklists for PR and OP applications for my fiancee. She is a Live-in Caregiver

For the Permanent residence forms:

1. Letter from Current employer showing start date and that you still work for them.
Does anyone have a sample letter for this?

2. Statement of earnings showing hours worked and deductions made by the employer.
we will be getting sheets from her 2 employers showing the hours worked, the amount she was paid and the deductions made.

3. Record of wages and deductions sent to Canada Revenue Agency (CRA by your employers.
What exactly do we need for this? I am unsure about this one.

4. Record of employment (ROE) for previous employers.
We have the ROE from her previous employer. One thing that I noticed is that it is not signed by her employer, do we need to get her signature on it? The whole thing is filled out and dated, but her signature is not on it.

5. Notices of Assessment issued by the CRA from the date you entered Canada under the Live-in Caregiver Program.
What exactly do we need to provide for this? T1?

For the Open Permit forms, do we need to fill out form IMM5710E? I cannot find exact info on which forms and what we need to provide for the OP application.

Please if someone can help us figure out exactly what we need to provide it would be an big help. Thank you.

1. Here is the sample of letter my employer wrote.

To Whom It May Concern,

Since ....... (starting date of employment) until the present day, ......... (name of live-in caregiver) has been employed in our home as the live-in caregiver for my children, ........ (name of children the live-in caregiver looks after). She is still currently employed by my husband, ....... (name), and I. Should you have any questions regarding .........'s (name of the live-in caregiver) work history in our home or present employment as live-in caregiver, please contact me.

2. I submitted copies of all my payslips for the time I worked which was for 24 months prior sending my PR application.

3. I submitted the copy of T4.

4. It would be better if it is signed by her employer and the employer's name and contact number are written on it although I didn't have any experience on this as I only worked for one employer.

5. In my checklist (3 years ago), it was written that we were required to submit Option C Printout from the date of entry into the live-in caregiver program which I could obtainnfree of charge by calling CRA number 1-800-959-8281.

For the OWP application form I filled out the APPLICATION TO CHANGE CONDITIONS, EXTEND MY STAY OR REMAIN IN CANADA AS A WORKER and next to this head caption I wrote in capitals (OPEN WORK PERMIT).

5. Notice of Assessment is the notice we receive from CRA several weeks after we file our income tax return.

Thank you for the detailed info, it really helps. I will make her an account on the CRA website, that should have all the info and copies of what I need.
