PPR - Appendix B attachment - Where is it?

Hi,

I got a PPR today. In the instruction, I need to sign Appendix B attached in the email. and send it to OTTAWA.

However, there is no attachment but 2 for images.

Could anyone tell me what it is about or alternate link for Appendix B for PPR?
