Do we have to list all entry and exit in to Canada as well in travel history? I have done short trips to my home country and came back to Canada. Do we have list all in travel history section?

You don´t have to list the travels between your country of residence and your country of birth...all others yes

Could anyone confirm if my understanding regarding EE travel history requirement is correct?

If my country of origin is Country A, and my country of residence is Country B, I traveled between A and B a few times in the last 5 years, and I also traveled to Country C once in the last 5 years.

The only entry I should enter in the travel history form is the trip between B and C, is this correct?

I believe your understanding is correct only if Country B happens to be Canada. If not, you'll have to provide the details of all travels between A<->B and B<->C.

Does your personalized checklist give any clarity on this front?

I don't think you are right about the "country of birth" bit. You mean to say country of citizenship.
