Go to your nearby PSK. (Passport Seva Kendra) center and enroll. and go to your passport mention address after 1-2 days for your verification as some documents needs to be signed by you. (they will always call before coming)

If you cant go, you know what to offer police men at the time of their visit.

I have been in Bangalore since April 2016. My passport is from Hyderabad, issued in 2010 (so I stayed 6 years in Hybd).

QUESTIONS:
1.Can I get the Police Verification done from Bangalore? Or I need to do it from Hyderabad only?
2. For Bangalore, I have postpaid phone bills and Bank statement (ciitibank, a private bank), rental agreement (am staying on rent). Will these documents be sufficient, if Bangalore is a possibility?
3. My ITA has not arrived yet, can I still go and initiate a police verification request? How do do this?
4. What all they check in Medical exam? I have been a smoker since 8 years almost now. Does that affect my chances of passing the medical tests?

You can get it done in Bangalore. You can refer to http://passportindia.gov.in/AppOnlineProject/popuponline/AttachmentAdvisorSub?subDocID=7001&confirmDOB=7001 for list of accepted documents. Postpaid bills might be sufficient for address proof. I don't think private Bank statements are accepted (you can double check on this though).

Btw, what do you want the police verification for? If it's for PCC, don't do it till you get ITA. Once you get ITA, you can book online appointment for PCC in passport website. You might need to take your passport + your ITA printout + local address proof to the local passport office (in Bangalore) once your appointment is confirmed. They will initiate a police verification if required.

I wouldn't sugget to go for Medical exam before ITA either. You'll have sufficient time to get your medicals done after you get your ITA. Smoking shouldn't be a problem unless you already have cancer.
