Police Clearance certificates

Hello Friends,

I have two questions on Police Clearance certificates:

1. The FBI form (http://www.fbi.gov/hq/cjisd/forms/cover_letter.pdf) has provision for only person person. I need to submit certificates for my wife and two kids. Should I submit one form for each person?

2. Does anyone know how long it takes to receive back the certificates? As you know, there are only 120 days to send the complete application package back to Buffalo office.

1. yes. please fill in seperate form for all and put in single envelop.

2. It will take 13-16 weeks to get the certificate.

Per form is per person and so 1 $ each.
