I am living in Coquitlam BC since 30 Nov 2107 up to now, and never leave Canada for 3 years and 9 months I have got refused pr profile, I was applying for a medical exam and police certificate from my original country both of them past more than 1-year also I have got ITA recently question is do I need to apply for a medical exam and police certificate again ?

Okay then. The medical and PCC are all valid for 12 months starting from the date of issue. So be the judge yourself and see if the dates are okay on em.
