I called BLS and they said they require original copy of work permit or notarized.
As I mentioned earlier, don't just follow the check list. Call them before you send your application.
They need more photos than what check list says and some other form which is not mentioned on the checklist.

10 photos , wow ... thats crazy even immigration and any other agencies doesnt ask that may .

can you please list what are the other forms that we need to send and how much will it cost for the entire process

1 police clearance certificate form.
5 copies of particular form signed.
3 copies of reference form.
10 photos of 35mm*35mm.
Original Passport and original Work Permit or notarized copy of work permit.
A draft in the name of BLS International Services Inc. for $40.40
For courier services another draft for $20 by regular mail or you can pay $35 over the phone by calling them. This $35 covers pickup and drop by fedex. (if you are using postal services)
The above mention list is for Vancouver Jurisdiction. Its different for Toronto and Ottawa. Please call them before sending your application.

I applied at Vancouver BLS and got my PCC in 10 days. If I remember correctly, I submitted only 1 or 2 photos. BTW I submitted my application in person. Its always best to go to their office and give application there. They are very helpful.

yeah my friend did in March and had to go by the old rules with less photos.
They've changed it now.
and I don't live in Vancouver. I would have to drive 3 hrs to drop in. So I used postal services.

I applied for mine in Montreal BLS, received it in 5 days.

yeah my friend did in March and had to go by the old rules with less photos.
They've changed it now.
and I don't live in Vancouver. I would have to drive 3 hrs to drop in. So I used postal services.
