i took IELTS on June 21, 2014 and got 6.0 overall, i-e R 5.0, L 6.5, W 6.0 and S 6.0. I am confused how to calculate my CLB level. Any advise which CLB level did i achieve? Need to apply for FSW in this week.

Actually you are ineligible - all four scores must be above "6" and your reading is too low. Must re-take the test again.
