Yes, when our medical expires, we have to re-do our med test.
With regards to visa issuance at the last minute, as i read in previous forums sometime back, they are really forced to enter canada regardless of any reasons.
Perhaps the best thing is, as soon as you receive your PPR, do the fastest action to secure your exit pass. Yet, in case you receive your PPR and you're sure that you cannot catch the time, then write to the embassy regarding your case before you submit your passport for visa stamping because when the visa is sticked already in your passport, I think you have a very less chance not to go before your medical expires.
