Any tips - exams undertaken, preparation (if you were to do it again, what you would have done differently), which doctor/clinic is best to go, etc.

According to file transfer policy of CIC, a visa office should not transfer the file to another visa office unless the transfer is warranted for program integrity reasons. Leaving a visa office's territory alone does not warrant a file transfer.
You can see the file transfer policy of CIC in OP 1, sections 5.19 and 5.20 and subsequent amendments.
Even if your file was transferred for any reason, the processing time in Cairo is faster at 11 months compared to 22 months for ADVO![]()
