No. I got my Medical exam pass around May.

No, my MR was April 13, 2018 and passed May 1, 2018.

Ecas just added the 4th line yesterday. Does that mean anything or was it just an update ?

Did you get an extension of your medical?

I didn't order notes but I think they must did. I called IRCC like 3 weeks ago and Agent told me do not need another medical, they have authority to extend my medical for another 6 month.
