I think you only need your passport.. some medical also ask for workpermit and UCI.

I am not sure about that. They asked me for my passport.. you can call them and ask..

No. The eMedical system requires passport information to be entered and then you must present your passport and recite details at every part of the appointment (bloods, xrays) to ensure it is actually you and not someone healthier that you sent in on your behalf.
