Yes, That's the website you should always use to download latest forms. Sometime other website forms might be outdated hence should always use CIC website.

There is one thing I'm confused about when reading thru the document check lists which is ppc requirement.

On the document check list, it's noted that police certificates are generally considered to be valid for one year from date of issue.
However, on the CIC website, under application guide, it's said "The certificate must have been issued no more than three (3) months prior to submitting your application"

I'm not sure which information is correct as I'm having my police check certificate 5 months old. :-[

Both information are correct. PCC is valid for a year and while sending your application OR when requested, you must enclose a PCC which is not more than three months old.

That means your five months old PCC is of no use now. You can send your application without PCC and provide LATEST PCC (no more than three months old) later when they ask for it.

Its mixed information in guidelines vs in other pages. I would say you go ahead and include the ones you have now before 6 months expiry and see if your visa office asks for it. Also parallel you should apply for new pcc now so that you can send it to visa office once you get AOR.

For countries where you have lived for six months or more, the police certificate must be issued after the last time you lived in that country.
