I got the PPR email on my gmail account and I couldn't see the Annex A until I clicked "view entire message". The Annex A is a table with your UCI info and you need to write the address that you want them to use to return your passport.COMPLETE ANNEX A FOUND AT THE END OF THIS DOCUMENT AND SUBMIT IT WITH YOUR DOCUMENTS.
