This is when an immigration consultant would have mitigated this issue. I am glad I used one for mine.

Your full name
Date of birth
Sex
Mailing address
Phone number (and/or fax number and/or e-mail address)

Do you have any finger printing services there?

I think we don't need Canadian polick check if we have already work permits in Canada. If CIC ask us to submit Canadian polick check we will do that. But basically we don't need.

You do not need a police certificate for your time in Canada if you are applying for permanent residence from within Canada in one of the following categories:

live-in caregiver
humanitarian and compassionate considerations
temporary resident permit holder
spouse or common-law partner
orprotected person or Convention refugee.

If you are applying under any other category or from outside of Canada, you may be requested to submit a Canadian police certificate for your time in Canada.

CIC website also says

If you have lived in Canada for more than 6 months after the age of 18, a background check may be automatically conducted with the Royal Canadian Mounted Police (RCMP). If your name has a 50% or more match in their data base, the RCMP may request fingerprints in order to confirm your identity.

I'm April SINP PR inland applicant.
14. January I get SINP Nomination.
27. April CIC received my PR Application.
22. July CIO send me UCI and EP number
9. November I received reminder mail from CIO and I pay RPRF.
From since nothing happened and no MR of course.
Considering my work permit expired in April 2016 I decide to do upfront medical.
I was contact panel physician from CIC web site and made appointment for medical exam. I told them that I have UCI number but not yet Medical exam request from CIC. They check and they said that they not have my UCI number in system but no problem to do upfront medical exam for PR.
 
M bcpnp applicant. I applied on April 10th. My husband received his medical request today. However I didn't receive it yet. May be very soon I vl get it.
