Hello Guys ,

Do we have to specify the FSW category while applying or does the CIO determines this from the forms /checklist?

I think they determine it from the application sent. They can easily find this out from the documents checklist form IMM5612 which we sent with application as cover letter because if you are FSW1 applicant you will not tick arranged employment section nor attach any relevant document, same in the case with category 3 so it's as easy as it is.
