Dear all I want to ask about filling IMM 5612 the document checklist form shall I tick all squares in the form or I just tick the document which are only compatable with my case for example there is one requirement ARRANGED EMPLOYMENT whic i do not have it in this case I have to tick the square beside it or I have to leave it blank as I do not have job offer in Canada or I have to do something else please please advice me anybody know exactly the answer please advice me thank you

Leave the square blank which do not compatible for you. It does not support anything than tick .Just tick the square which you are providing on the application package.
 
ok in case I do not tick the square should I submitt a written explanation like use of representative I write in separate papaer that I'm applying by myself and not hiring a representative

I did not touch the sheet of representative....

Oh I see. In that case, I only ticked those items which I sent.

not necessarily. I did not do that. But are you skipping any important document to send? If you believe, you are sending all the document you are sending, no need to confuse visa officer. Document checklist does not mean that you have to send all, just tick and send what you require to send.
 
Right Deba, I thought last night its application form.
its ok, but don't forget to send both checklists : CIO & CHC checklist. whatever your documents are just tickmark those parts, do not select if that section is not belongs to your documents check list.
just arrange it as per CIO and CHC check list.
you can write seperate letter for more instructions if any document/s is missing (like some countires PCC's take time so you can write it that you will submit it later)

I want to ask last question is there CHC for visa office london now I think they are not asking for it now like before the checklist showing it only for people applying fron China is it true
