After the examination

- However the DMP will also provide you with the photocopy of the completed medical examination forms for each applicant. You should submit these to our office, along with a photocopy of the passport which was presented at the time of examination for each applicant.


What form is it referring to ..when it says "completed medical examination forms" and what photocopy of the passport "You should submit these to our office, along with a photocopy of the passport which was presented at the time of examination for each applicant."

For me, DMP didn't do anything on the passport. What photocopy it is refering too? Please let me know

I thought the instructions were self explanatory....
- the first part means when you complete the medical forms, the DPM will give you a copy of the forms you have completed
- the DPM will request for your passport for verification purposes. Basically you send VO photocopy of the med forms and photocopy of ur passport....
This is my undersstanding of your post.
