Im a Canadian citizen by birth looking to sponsor my long term girlfriend as common law, through family class.
Almost completed our application, but went over checklist and nearly missed form IMM5475 !

it says If applicable?

do we have to do this form? thanks!

If you want to release information to someone, yes, you fill it out.

Ok so if I put N/A that means only me the sponsor can get info regarding status. If I put my spouse for example she can get updates? thank you
