In the generic Application for Permanent residency it asks in question 4. Immigration Office requested for processing this application. Do I put where I am send it??

Also in the Sponsor Questionnaire it asks are you currently living with your sponsor and I put no. But next to it, it asks when did your sponsor return to Canada. What am I suppose to put there?? I lived with him for the full year in canada before returning to america.

You should put the immigration office located closest to where the applicant is. So for mine since my wife is in the Philippines I put in Manila.

Don't know the answer to this one, I don't have it on my forms. What's the app number for that one?

Edit: Found it but it's a different situation for me as I was in the Philippines and she wasn't in Canada with me. i just put the date that I flew out of the Philippines, don't think that will be applicable to you unless the sponsor visited you in the states. You may be able to put in the date you left Canada and then attach a note explaining it. Hopefully someone can give you a better answer than me as I'm not entirely sure.

Thank you for answering either way. :)!!!! It gives me a good idea on what I should do. I was also wondering when it asks to provide documentation in the places you write does it mean I should write down all the proof I have or can I just scan the papers and send it with the application??

I scanned and sent my stuff in for the most part.

I also added a printed letter for each application form with notes on anything I didn't think was clear or that they might have questions about. So for each one i would have a heading of IMM XXXX and then in point form below I wrote out an explanation on anything that might not be obvious. I also did this with the checklist, so for the police clearances I made a note that we already filed for them and would submit them with the application later.

Another thing I included was a 3 page word document outlining how we met, how our relationship progressed as well as details of my trips to the Philippines and wedding. Anytime i mentioned a plane ride or expense in the letters i also included a note that the receipt or picture is included in the application.

Basically, anything you don't think is clear or you aren't able to fit, document it and send it along with the application.

I take it you are the applicant (the non-Canadian spouse). In that case, you have a choice between the visa office responsible for:

1) your country of citizenship; or

2) the country in which you live, if you've been legally admitted to reside there for a period of at least one year. (For example, if you were allowed in on May 17 with permission to stay until May 17 next year, then this counts.)

If these are different, you should check which one has the shorter processing times and probably apply to that one.
