PLEASE HELP - Which IMM5669 BACKGROUND CHECK form is the correct one?

Hi Guys,

My wife is sponsoring me and we live in Canada, so we sent our application since April 2018 and I received my CSQ on October 2020.

Now the federal government is asking me to fill out the backroun check form however I am confused.

The form that is in the sponsorship application package is named ''eimm5669e'' dated from 11-2018: (https://www.canada.ca/content/dam/ircc/migration/ircc/english/pdf/kits/forms/eimm5669e.pdf)

whereas there is a new one that is dated 06-2019 and is called ''imm5669e''.

Both are different but I don t know which one to choose, normally I would pick the recent one but he is not in my application package so I don t understand...

If u were just submitting your package i would have advise u to send the one that come with the check list, the latest imm5669 is okay to send if it is a requested document
