PLEASE HELP: Received PPR today but Appendix A form is not attached

Hello, if anyone has experienced anything similar? My husband received PPR today but there is no Appendix A form attached with his PPR email. how do we now submit height and eye colour?

Just on a piece of paper, any simple table format will do, name DOB UCI and height/eye colour should be enough.

(I did eventually find the formal Schedule A and it's just a simple table. VFS said we could just write it on a piece of paper but there were several of us)

If by issue you mean that the instructions say schedule A but they don't have it, yes. No real issue - just submit as I explained above.
