1. POF which is 2.5 years back (meaning literally more than 6 months).. Do I need to show the source of funds??

2. What can be the official document to prove Name change?? Please consider name change of my wife after our marriage.

I m not sure for 1st but for name change I attached explanation letter with my wife's birth certificate, marriage certificate and a current passport having my name on it.

You will likely need to get an updated letter from the bank, or something more recent than 2.5 years ago. CIC has no way of knowing if the funds listed on the old document are still available.
