No. not the application form. it was a one page form given to me when the application submission was being finalized.
Edit: It is not done online, it was in CHC, after submitting the documents.
the problem is following:
I applied for a visa on June 14th that got refused on 23rd. Reapplied again today. The first time, after submitting all the documents they gave me another form to read and fill up. That form had some statements under which I had sign and put date. And there were three questions, for example 'have you ever been arrested or charged for criminal activities?' (something similar) with the answering option 'NO/Yes (if yes, explain). There were three, one about this arrest thing, one about racism, sexism and stuff like that but the other one I don't know about. At the time of submission I remembered how last time i read them and checked 'no' for all of them so did the same this time without reading it (i know i'm stupid but let's move on
). Now my worry is whether that last question was regarding 'past visa refusal'.
