Please help, can't find Schedule 1 Background Declaration

Hi all,

I was requested to submit Schedule 1 Background Declaration form but I can't find it on CIC website. Could someone help me?

Should be a typo. Background declaration form is Schedule A. Does the request letter mention any specific timeline?

Hmm... If it's a typo, now it's unclear what to do :(
About timeline it says: The above information/documents must be received in our office within 60 days from the date of this
letter. Also, I couldn't find a space on MyCIC to upload this information. I guess I will send it with courier along with original PCC that they requested.

Send it in along with CSE. Have you checked your post ITA application completely? Document checklist gets additional upload placeholder(s) when CIC sends a document request.

I just checked the CIC webpage for the list of Forms download page and it shows the below as Schedule 1. Its Form IMM5257. Its a two page form named APPLICATION FOR TEMPORARY RESIDENT VISA. Does it ring any bell?

Regarding upload placeholder my post-ITA app looks like this:

Even when you continue beyond this page to document checklist?

Anyways, if you're unable to locate the upload placeholder send a CSE asap within the prescribed deadline.

Ah, I'm sorry, was my stupid mistake. If I continue, I see the page with upload placeholders. And it has 2 uploads: for PCC and for Schedule 1 form (it even says Schedule 1 - Application for a Temporary Resident Visa Made Outside Canada (IMM 5257) and leads here http://www.cic.gc.ca/english/pdf/kits/forms/IMM5257B_1.PDF)

But my another concern is: so should I just upload PCC here? or send with courier? Here is a full letter that I received:


Dear XXX:

This is in reference to your application for permanent residence in Canada. In order for us to continue the
processing of your application, we require the following documents:

>UK Police Certificates for yourself and your spouse: provide an original police
clearance certificate This must be received at this office by: 2016/03/21

> Schedule 1: A separate and newly completed Schedule 1 Background Declaration
form for your spouse . Personal history and addressess information must be completed since his

All documents must be accompanied by English or French translations. When original documents are
requested, notarised documents are not acceptable.

You need to upload all your documents here be it PCCs or Schedule 1 or A (Whatever).
