section A name nung applicant then young name nung spouse whoever then mother,father info ng applicant (dont sign below)
section B pag walang children leave it blank but sign it below to certify that u dont have children
section C brothers and sister info nung applicant including half bro and sis
section D ( signiture ) ng applicant very important!! and always put a DATE
kailngan lang naman nila yan just incase in future na mag sponsor ung applicant mo ng member of the family naka declare na .. so if i were u DO IT!
