i scanned my photos and pasted them in work. is that ok? i have 3-4 pictures in 1 page. i have included the people in it, the date, location and a brief description.

question, what do i write on top? just my name? example: applicant: XXXXX that's it?

is there like a number or client id need?
 
I'd put applicant and sponsor info (names, addresses, DOBs) at the top of your document.
 
When it came to the photo proof - I, like you, arranged 4 scanned photos on each sheet of paper adding the occasion, date, location & names of people in photos under each picture.

I didn't label the pages, though. However, I put all the photo proof in a separate transparent folder sleeve and labeled it stating our names (me and my bf) and the time period when all the pics were taken.
 
we just printed all our photos out and put no labels on it... ours got approved in less then a month in london office.. i dont think they are actually that particular about things :)
 
I included around 75 pictures, as I had visited my wife 3 times, pictures of our wedding, trips, meeting her parents, etc. I numbered each picture, dated, and said who was in it / location using word and Avery labels. I numbered them so they would know if one went missing and also so I could refer to pictures in the answers to some of the questions.
