PhD/D.Phil. applicants-Work ex. documentation

Hello, anyone who applied with a Ph.D. or have an idea of what documentation to submit. I was working as a full-time paid TA (NOC 4012-A) during the Ph.D. but need help around the document to show it as work experience. From what I know if I can get a letter from the university saying it was paid employment with regular pay it should be sufficient for CIC. Anyone who has done this or has an idea of the letter template?

There is a thread about 4012 applications, search the forum. In terms of the template, I think CIC website has detailed instructions on what information should be included (e.g. salary, hours worked per week, job title, letter head, contact information of advisor, job duties....), do some research online.
