I don't think it matters who at the company fills out the LMO as long as that person is approved by the management to do that. To get an LMO, they also have to show that they have advertised the position at the job bank for a certain period. And actually, you don't need to prove recruitment efforts since you were on a PGWP ( Might be helpful to enclose a copy of your PGWP). If you do, it won't hurt, tho.

Note that HRSDC has developed new application forms, effective March 25. I can't get the new forms to download, but you should try. I don't know if they will accept applications on the old form...
