Personal Particular Form for PPC from SF consulate in USA

I am very confused about filling in the Personal particular form. I am an Indian citizen(passport issued in India not from Indian embassy in US- coz its different if the passport is renewed/issues in USA) residing in the US for 5 years ( and now applying for Canadian PR) not sure what to enter in 8(b) and 9 and 10.

Has anyone been in the same situation and got PCC from USA done successfully- for passport issued from India not from consulate in USA?

Even though you have lived in the USA for the last 5 years, your permanent address is still in India. The form is very straightforward. In 8, your address in India. In 9, your current US address and 10 address of where you work.

I don't see where you got your passport issued having any bearing on the answers in the form.
