Does anyone have the sample of personal information form for principal applicant ? Is there a different one for spouse? From where to download it?

Noo i am really sorry. I didn't mean this form. There's a form to fill in our details like personal information, family information,work history, personal history wherein we are supposed to fill our past 10 years history including all dates, etc. I mean this form. Or is it a different one if we are submitting it online? I met a consultancy and they gave this form which I had to take print out and both the forms for principal applicant and spouse looks similar. I am not sure about this and want to clarify. Pls help if you know about it.

It’s called Schedule A aka background declaration.

Note that if you are submitting online, you get personalized forms you need to complete which includes questions from Schedule A. So the consultant probably wants you to complete the hard copy so that they transfer the info to the online application as you don’t have access to your CIC account.

hello everyone,

i have a question about Schedule A form


i have received my ITA, and i have to submit the documents within 30 days now,

i have a question, my masters degree is through a distance learning program(DLP), while pursuing masters through DLP, i was permanently employed in a company. how should i explain these two years of job and study in the last 10 years history form?

should i attach a Letter of Explanation (LOE) with my documents and explain the whole scenario, as the university is in a different city and i was employed in a different city? as it was a distance learning program?

will they count this experience as full time employment or this will be considered as study period? or the experience and degree both will be valid as it was outside Canada

Can someone please explain this
