You can pay it at any time, as they won't issue COPR (visa) until you do.

hi, when you did the medical is because CIC send you a letter? or it is because you wanted to do? in my case when they send me the list of the documents i should submit, the medical and the Permanent Residency fees requested the same time, i believe you should until you are notified...

don't worry, CIC will tell you when to pay the RPR fee
