Do I need to provide PCC for every address we have lived in the SAME country for past 10 years or just the recent address????

Apply at PSK and you should be fine. One PCC covers whole india. The other alternative would be to apply at local police for each district you lived in in the past 10 years.

I changed the address within the same city, Even within the same Police Station Jurisdiction.

Just apply through the passport office. They’ll do the rest.
