Passport Scan question

In my previous applications to CIC (off-campus work permit and post-graduation work permit), I have included in the scan of my passport only the visas and stamps made by Canadian officials. For the PR application, do I have to include the other visas I have there (my US visa with two stamps which will verify my travel history, and my Egyptian visa which I got when I had a vacation there before I turned 18)? If so, do I have to write anything in the explanatory note about this?

Yes, at least scan all pages with biases or stamps. It's good practice to scan all pages, stamped or not.

It should be self explanatory. The VO knows your birthdate and the reasons you wouldn't include that information in your history. It also doesn't hurt your application to reiterate that it occurred before your 18th birthday.
