PASSPORT REQUEST- MANILA

my father in law got a passport request for his visitors visa application... can someone please clarify how much managers cheque he needs to pay with his passport

is it 285php + 537 php only

please help po to anyone that has done this.. also can you please let me know what else do we need to include besides the self stamped courier envelope for the return of the passport, plus the letter requesting the passport is there any other forms
