Passport application form all pages

Can someone please let me know if I need to include all the 7 pages from the passport application form or just the signature pages for mail in application?
Also, When my guarantor signed the form, the version on the bottom is 03-2022 but now the version changed to 11-2022, can I still use 03-2022 version if I mail it today? It is a big hassle to again get them sign everything and send back.

You don't need to include the instructions pages towards the end of the form. Ideally, don't use the old version. Chances of rejection are high.
