Parent's Surname in Passport is printed wrongly - Affidavit needed?

Hi,

Both my parent's surname has been printed wrongly in my passport. Do we need an affidavit for this? I have this question, because we need to enter parent's name when we submit family details.

Also I am claiming points for Sibling relationship proof, where my parent's name is correctly mentioned in by Sibling's passport. Any feedback will be great.
