Provide the details of your personal history since the age of 18, or the past 10 years, whichever comes first, in chronological order. Do not leave any sections blank or gaps in time. You must write "n/a" in any sections that are not applicable to your situation. Include all:

• being a homemaker.

Travel documents and passports

Passports or travel documents for you, your spouse or common-law partner and your dependent children. Include only copies of pages showing:

the passport number,
date of issue and expiry,
the photo, name, date and place of birth,
pages showing any amendments in name, date of birth, expiration, etc.
You must hold a valid regular passport. Diplomatic, official, service or public affairs passports are not valid for immigration to Canada.

Proof of Admission (if applicable)

If the visa office you have specified for processing your application is not the visa office responsible for your country of nationality, provide proof that you have been lawfully admitted for a period of at least one year to a country for which the specified visa office is responsible.

Proof of lawful admission is in the form of a stamp in the passport or a status document issued at the Port of Entry or other place of admission. If an extension of the original status has been obtained, the evidence required would be a document or stamp issued by the ministry responsible for immigration matters, providing an extension up to one year or beyond.

Please note that the CPC-Mississauga is not a visa office. Visa offices are outside of Canada.

Submit photocopies of:

• current or expired entry/exit stamps in your national passport, and

• current or expired status documents indicating the date you were admitted and the validity.

Submit proof of relationship showing you are the parent or grandparent of your sponsor in Canada. The document you submit must prove the relationship between the principal applicant and the sponsor.

National IDs, family/household registry/book (if applicable).

You must provide the following documents for you and your current and/or former spouse/common-law partner:

Marriage certificate(s), final divorce or annulment certificate(s). If married more than once, include certificates from each marriage and divorce or annulment you, your spouse or common-law partner have had;

I also recommend everyone to make sure they also pay the RPRF fee for everyone involved in the application, to save time/hassle later on in the process. (if possible)

Just so no one is mistaken when reading this few months/years down the road, the list posted by tdolphin is upto step 8 and doesnt include 9 to 11. Just wanted to point this out because these are delicate matters and precision is warranted.


Since it takes time to get a police report(depending on the coutry), is it OK to send it later when requested? Does anyone know what the process is and how long it takes after they receive these documents and approve the sponsor?

The call centre is fully aware of the missing steps.Call and provide your email address they will send it to you the same time that you are on the phone.They email it to me even without me calling them as I had provide my email when I applied 4yrs ago for my mom.

I have a question? My parents lost their marriage certificate which is almost 40years old. Will that be a problem? Would like to know where is ur parents file getting proccessed? Would appreciate if you or anyone else can help.......

I got the same exact thing - steps 8-11 are printed twice. Doesn't make sense although I am thinking of calling CIC but not sure if the Call Centre folks can help with this as "apparently" they have the exact same info that is available through the online status check.

Anyone else that has actually gone through and had a similar situation? Any lawyers out there?

same here, my parents don't even have a marriage certificate. I m using there bank statements and other docs where they have name of spouse. In there home country, in passport it states name of spouse.

Hi, Does anyone knows how long approximately it takes after u submit the request for Option C printout and parents documents. Well, i applied in March 2011 in Mississauga but i received mails for additonal documents from Ottawa. My parents live in India......does that mean after sponsorship is confirmed the file will be send to New Delhi and if yes how long? Please let me know if anyone has a more recent timelines.

My parents visa centre will be Islamabd, Pakistan. As for the 2nd question, can they get a copy from the local regisry office of there marriage certificate because although they lost it, there marriage is still recognized and recorded by the state. I am not sure if bank statements can be used to proof that as someone mentioned but again I am not too sure. Reason(according to my thinking) being what if the bank in your country allows to open a joint account for unmarried couples?

Missing Steps Update:
This morning I called CIC and the call centre acknowledged that they are aware of the problem and emailed me the complete checklist.

Ru sure its from ottawa coz we all got it from Mississauga?

I have the same problem . my parents dont have any certificate. my parents are going on sat to make one. Will keep you updated once I get more information.
