yeah, the question is, when two forms filled who is applicant and who is spouse?
when you fill spouse or co-applicant, who do you use as applicant? In my case I used co-applicant name as applicant as co-applicant is the one signing the form.

i havent' received the letter from cic yet - so not sure if i filled in the forms correctly...

my assumption was that applicant means principal applicant... so it is only one person for application package, not both parents... in my case - my mother...

so in IMM 5604 for my mother we wrote her name as applicants name, my fathers name as a spouse, my name as a child and my mother's parents + sisters/brothers...

in IMM 5604 for my father we left the applicant name blank, wrote my fathers name as a spouse, my name as a child and my father parents + sisters/brothers...

I consult with my friend who works as a immigration consultant. Here, what he has mentioned me.

You can fill out this form two ways.

Option 1:

You can fill out one copy with applicant name and fill out mother, father, son,daughter, brothers and sisters information of applicant... and on a second copy you leave applicant's name blank and fill out mother, father, son, daughter, bothers and sisters name of co-applicant.

Option 2:

You can fill out forms using each others name in applicant and spouse and fill out forms accordingly.

I hope these helps.

I think the reason we have to fill out different forms for each persons, who is included in the application (Father, Mother, and dependent child who is 18 years and over) is cause when we have to fill 5406 for dependent child who is 18 years and over, how can we put his name and other information like in Section B and Section C if we are putting his name in Section B

So I think each person have to fill out his/her own 5406...

You are welcome brother.... I am still in a waiting game... My application was received by CIC on January 13 2012... I think I have a long waiting time... But I attached PCC along with all those documents....

sponsorparents- I actually didn't do either of the options. I put my mother's name as principal applicant and my father's name as the spouse in both the 5406 but I put information specific to my father's brothers and sisters in one and then for my mother's brothers and sisters in the other; children information remained the same in both. I think that's what caused the confusion as at a quick glance both the forms might appear the same as it begins with the same applicant name and spouse name. However, I had put them separately and also attached a table of contents (!) which mentioned that there were two separate forms....mediocrity and CIC go hand in hand...ugh!

@smodhera- many thanks for sharing the information from the consultant...very helpful.
@supervisa- since you have already received the approval, can you share how you filled IMM 5406?
