I have a question on form IMM 5669
My dad was sem-retired for full year (2017), I have marked that year 'unemployed' under personal history. During that time, he travelled to England with family for 1 month.

As per the guide on IRCC website:

If you were not working, provide information on what you were doing (for example: unemployed, studying, travelling, retired, in detention, etc.). If you were outside your country of nationality, indicate your status in that country.

Now, I am confused, should I enter this month trip as well within the Personal history section?

Also, for my mom's Additional Family Information (IMM 5406) form, I don't have enough space to enter details of all his brothers and sisters. I have to attach extra sheet to put in the remaining information. Does anyone know the format of this extra sheet ?
 
Guys I have one question, I am just stuck with the solution for birth certificate. Indian birth certificate is in Hindi & has only fathers name and my dad just passed away last month. So I will be showing his death certificate as proof since I had put both my parents name in Interest to sponsor form. For my mom now what can I provide? Can I just send them a copy of my passport showing both their names and maybe notarized here along with this being a true and certified copy? In addition could I put in my mark sheets from school that does show both of their names on it and is in English?
Do I still have to send a copy of my birth certificate after getting it translated and certified even though it doesn't have my moms name? Should I provide a letter explaining all this?
Sorry for a long message but will really appreciate the help!!!

For imm5669, what should we put in given name?
For the rest of the forms, I have used given name in last name field and left given name blank.


Please advise if its ok to put “not applicable” in imm5669 to validate it.
Hi there I need your advice on this please.i was also unable to validate the form imm5669 if i leave given name blank, as there is no surname on my parents passport only family name.

I used same form IMM5406 to add more siblings and marked it as page 2/2 and 1st form as page 1/2.
 
Check out previous threads.
If you know year or approximate year. Put that. Use 01/01 as month n date. Write an explanation letter. Refer to question 3 of sch a.
Validate. Sign form.

You can include affidavit and explation letter along with any copy of other documents where your Father's name is written such as passport, Educational certificates. Yes still you need to translate birth certificate to show relationship to your mother and to make it clear to immigration officer.

Do we need to certify or notarize photocopies of original supporting documents like marriage certificate, birth certificate? or we can just send photocopies with application.

Guide .. invite letter both say this form must be validated and signed. Same was said by ircc agent on phone. Unknown cant validate the form. Sending unvalidated form is too risky.
Approximating cant be unknown. U shld call ircc to confirm. 2 cents.
