Are we required to mention the time that we (sponsor) went for any vacation during last 3 years?
In the Q14 of 5768?

We need to provide Employment history only. Do not need to include vacations for Q14 in IMM5768 ,Because whenever you go to vacations still you are employed. If you were unemployed then you need to provide unemployment history.

I submitted original one when I applied for Visitor visa which didn't specify purpose of the affidavit and I kept a copy for my own records. Now for PR, I submitted new original one since the old one was expired (lawyers renewal was expired as per all stamps on affidavit). This one also does not specify the purpose of affidavit. It just says about spelling/name variances. Hope it helps!
When the names of parents varies between their passports and the birth certificate of the sponsor, who prepares the affidavit regarding the difference-parents or the sponsor or both sign it? My parents' surname is missing from my birth certificate.

My parents had been granted permanent resident 10 years ago, and they did not comply with the residency obligation ( reside 2 years in Canada in 5 years). When they applied permanent resident travel document to come back and got rejected. This year, i applied again.Should I put "yes" for this question. My understanding is that the permanent resident visa specified in this question is for the first immigration visa for landing, am I right?

As this Birth certificate belongs to you so in this case you need to prepare an affidavit. It is not necessary to include Parents signarues on affidavit.

  • bear the name and date of birth of the subject, as well as the name and complete address of the photography studio;
  • bear the date the photo was taken;
  • The photographer may use a stamp or handwrite this information. Stick-on labels are not accepted."

I had it made by my parents saying their name/spelling varies between their own documents such as birth certificate, passport, marriage certificate, etc. and also included their name in affidavit as it says in my passport and birth certificate.

PLEASE HELP/COMMENT.......I'm filling in the Schedule 1 for my father. His father's date of birth is in 1800 which the the form does not accept. The guideline says write it with hand if before 1900, however the form cannot be validated. What should I do? anyone had the same experience? Is it fine to send them the form which is not marked as Validated?

Handwrite, and include a brief-letter as to why the form is not validated. This form does not produce a barcode, so the Validated - Yes / No box is just for the Case Officer's assistance to make sure all info is completed on the form. This is unlike the two other forms where the Validate action produces barcodes which contains the form's completed info; which the CIC scans to input data in their system associated with your file.

In your case, s/he will read the letter, see "Validate-No" on the form, but will know exactly why it isn't validated.

Hi IMM5768 - Question 14 - Employment history of sponsor - Family size - current 3 (Me, Wife and Kid) ? or total with application 5 (Me, Wife, kid, Mother and Father). Kindly advise....
 
For those who can check status on ecas, can you link your application yet? Seem like a few can link already but I still cannot, so worried now

I am filling an additional family information form.

My mother has 6 siblings and i am going to attach an extra additional family information page to fill up section C.

Do i need to fill up only Section C in second page or the full form again?
