Parent Case: Medical request while in Canada

My mother lives with me in Canada. Parent sponsorship case was approved by Mississauga in June 2012 and file was transferred to the country of origin (Islamabad, Pakistan). This Monday (15 Dec, 2014) I received medical request via email. CIC's website clearly says you can get medical done anywhere in the world. Do I need to tell doctor that it was sent by CHC Pakistan and should be sent back to regional office (India) or they are smart enough to figure out where it needs to go to? Or all results go to central system so it doesn't matter?
Also, my Canadian address is what we have on her file as current address. Are they going to send Passport Request here in Canada? or we need to provide them local address in Pakistan. So far all communication has been sent to our Canadian address. (Case is handled by CHC Islamabad , Pakistan).

CHC Islamabad sent acknowledgement of receipt and request for additional documents April,2012

Med can be done where the applicant lives so no prob if your mother lives in Canada she can get it done in Canada and as it is online system your file be updated online.
I assume that now your VO is LVO as sponsorship cases are now being processed in LVO. If you get PPR and asked to send to CHC-ISB then you may request to change the VO as applicant is already in CA so I am sure will get an updated email regarding that. To be on safer end send an email to VO and tell about med being done at CA.

Thank you so much for your prompt reply. The medical request letter has Islamabad's address on it and it was my understanding that once your visa office has been decided you cannot change it later. I wont mind sending email or even a letter right away telling them due to situation in that country its not possible for a senior person , with no ties to the home country anymore, to travel there and stay on her own until stamped passport comes back but is it really possible to change Visa Office at the time of visa stamping?
Also I got GMCS notes twice during last 2 years and there was no mention about London Visa Office.
