Hello,

I am in the process of getting my medical check done.
I live in the Los Angeles area. In the past, I had someone advise me of a DMP who could complete my Panel Physician check.
However, on the CIC website, it only lists one doctor in my area.

Is this the doctor I must you?

Also, I just want to verify, I will be applying for permanent residency.
I will need to get my police certificates, correct?
I am applying in the family class as a spouse of my wife.

Thank you for your help in advance.

Also, if anyone can provide me of a quick link the form I need to provide my doctor, I'd be more than happy. I've spent the last half an hour searching for it on the website.

You must use a doctor on CIC's list of panel physicians. If doc is not there, you can't use him.

There is no form you need to give the doctor. Advise him it's an "upfront" medical exam, and after the medical exam is done he will provide you with a receipt with a reference number that you include in your PR application. I think you also need to bring in some photographs, but not sure specific details on this best to check your guide.

Yes you will also need your FBI police check. They are valid for 3 months from when you receive it, so make sure the app is submitted within this time.
