Salaam Bajwa bhai.
Sir it means that your cousin has to send original passport to CHC Islamabad along with the photocopies of passport and two photographs. And the Chc Islamabad forward the photocopies and photographs to CHC London and Issue the visa from Islamabad when instructed by CHC London. Means PPR 1 and PPR2 comes in same email.

But the question arises is that how come his e-cas becomes decision made before the issuance of visa. It means the CHC London has made the decision before the reuqest of his passport which is a good sign.
