We haven't got those amunaization forms with our corps ( many ppl did not ) any way according to my research ( asked some friends with small school going children)who already live in Canada , the schools at the admission time ask for the child's clinical amunaization card ( pediatrician's clinic ) and a letter from the pediatrician on his clinical lettre head in which he states that your child has been ammunized according to the Ontario child ammunization standards . Do ask your doctor if he has been using and prescribing internationally recommended vacsinations ( a lot of very good doctors DON'T do that here in Pakistan ) luckily my babies have been going to Dr Sajid Maqbool here in Lahore , he follows Those international ammunization cods ..( we really dint know that good aspect of him at all... Hahaha ) . He gave us a letter for each child saying this child has been (following and vaccinated according to the Ontario ammunization peel standards )
So I'd say talk to your doctor and ask him to provide you a letter from him, and take your child's ammunization card . That's all the schools are asking for over there !!!
