Send the email back to LVO and ask them about it, MR doesn't contain the names of your family members.
They will sent you the correct MR within 3-4 days.
Mine had some error and They sent me back the corrected one. Meanwhile get the appointment from IOM.

Hi all, i have received MR through email, but medical request form is attached only for me and not for other family members, is it ok? its from CHC London. I think form should be for all. Can any one tell that IOM can also provide these forms?

I am relocating from my current residence. What should I do? How much time does the passport take to be returned? Would they send it to the address or TCS visatronix????

AOA, well thats true CIC asking passport in 2 steps. Step 1) asking photocopies at London Stage 2) passport at Islamabad for visa stamp.
I have had my family medical done on 17th Oct-2012 & stage-1 received on 7th- Dec which i have sent & they received on 27 of Nov, since then I am waiting in the mean time we have had Pre arrival orientation done on 22-12-2012. so I believe after Christmas holidays they gonna ask me for stage 2...
